Permission to Medicate Student
Intent to Volunteer / Background Check Permission
If there is ANY change in your child’s usual dismissal procedures (for example....if he/she is going home early for an appointment or if your child will be picked up in any different manner than their usual, daily dismissal routine) please fill out this form and have the student hand it to his/her teacher when they arrive at school.
** This form is the safest way to guarantee proper dismissal arrangements. Phone calls, voicemails and emails do not always make it to the proper person if there are substitutes or circumstances that prevent this kind of communication.
